12-Hour Cancellation Policy Appointments canceled 12 hours or less before the scheduled appointment time will incur a cancellation fee equal to 75% of the scheduled service price. Appointments canceled more than 12 hours before the scheduled appointment time will not be charged a cancellation fee. Cancellation requests must be submitted through the Business's approved booking or communication methods before the 12-hour deadline. Late Arrival Policy A 10-minute grace period is provided for all appointments. Clients arriving more than 10 minutes late may have their appointment shortened, rescheduled, or canceled if there is insufficient time remaining to safely complete the scheduled service without affecting subsequent appointments. If your appointment must be canceled due to late arrival, you authorize Raw and Defined Beauty Bar to charge 75% of the scheduled service price to the payment method on file. No-Call / No-Show Policy A No-Call/No-Show occurs when a client fails to appear for a scheduled appointment without notifying the Business. Required Client Forms For the safety of all clients, all required intake forms, medical history forms, consent forms, waivers, and any additional documentation must be completed 4 hours before your scheduled appointment. Failure to complete all required forms before your appointment may result in your appointment being delayed, rescheduled, or canceled. If your appointment is canceled because required forms were not completed prior to your appointment, you authorize Raw and Defined Beauty Bar to charge 75% of the scheduled service price to the payment method on file. If information disclosed on your medical history or health intake form indicates that a requested service cannot be safely performed due to a medical condition, medication, skin condition, allergy, pregnancy-related concern, or any other contraindication, your appointment will be canceled for your safety, and you agree to pay 75% of the scheduled service price for the reserved appointment time. Clients are required to complete a new Health History Form every six (6) months. If your medical history, medications, pregnancy status, allergies, skin condition, or any other health information changes before the six-month renewal period, you agree to notify your esthetician before your next appointment. Clients who fail to appear for their appointment without notice authorize Raw and Defined Beauty Bar to charge 75% of the scheduled service price to the debit or credit card maintained on file. Repeated No-Call/No-Show appointments may result in refusal of future booking privileges.
